Required Account Documents

Everything you'll need to open a business account. 

We are glad you are considering us to serve your business needs.  To get started, we will need some information required by federal regulations.  The list below will help you to quickly and easily determine what is needed for your specific business.  Of course, if you have any questions we are happy to help you along the way. 

To help the government fight financial crime, the Federal government requires that we obtain, verify, and record information about the beneficial owners of our legal entity customers.  With the exception of Sole Proprietorships, a Certification of Beneficial Owners of Legal Entities Form is required for each business account.  Please read the form carefully and complete it according to the instructions.  For more information on Beneficial Ownership please read our Beneficial Ownership FAQs.

Sole Proprietorship

We will need the following to open a business account for a Sole Proprietorship: 

  • Form W-9
  • Assumed Name Certificate
General Partnership

We will need the follwing to open a business account for a General Partnership: 

Limited Partnership (LP, LTD)

We will need the following to open a business account for a Limited Partnership: 

  • Form W-9
  • Partnership Agreement
  • Assumed Name Certificate, if applicable
  • Certificate of Filing and Certificate of Formation (or Certificate of Limited Partnership if formed prior to January 1, 2006)
  • Business documentation for the general partner (if the general partner is an entity)
  • Certification of Beneficial Owners of Legal Entities Form
Limited Liability Partnership (LLP)

We will need the following to open a business account for a Limited Liability Partnership: 

  • Form W-9
  • Partnership Agreement
  • Certificate of Filing and Certificate of Formation (or Certificate of Limited Partnership if formed prior to January 1, 2006)
  • Application for Registration as a Limited Liability Partnership and the letter of acceptance from the Secretary of State
  • Assumed Name Certificate, if applicable
  • Business documentation for the general partner (if the general partner is an entity)
  • Certificate of Account Status/Franchise Tax Account Status (often referred to as Certificate of Good Standing)
  • Certification of Beneficial Owners of Legal Entities Form
Limited Liability Company (LLC) or Professional Limited Liability Company (PLLC)

We will need the following to open a business account for a Limited Liability Company: 

  • Form W-9
  • Certificate of Filing and Certificate of Formation (or Certificate of Limited Partnership if formed prior to January 1, 2006)
  • Assumed Name Certificate, if applicable
  • Operating Agreement (often referred to as Regulations or Company Agreement)
  • Business documentation for the manager or managing member (if the manager or managing member is an entity)
  • Certificate of Account Status/Franchise Tax Account Status (often referred to as Certificate of Good Standing)
  • Certification of Beneficial Owners of Legal Entities Form
Corporation (Inc. Corp.) or Professional Corporations (PC)

We will need the following to open a business account for a Corporation: 

  • Form W-9
  • Certificate of Filing and Certificate of Formation (or Certificate of Limited Partnership if formed prior to January 1, 2006)
  • Assumed Name Certificate, if applicable
  • Bylaws (often referred to as Regulations or Company Agreement)
  • Certificate of Account Status/Franchise Tax Account Status (often referred to as Certificate of Good Standing)
  • Certification of Beneficial Owners of Legal Entities Form
Social Organizations, Unincorporated Entities, and Civic Groups

We will need the following to open an account for an unincorporated entity:
